Coverage Plans
Insurance
Insurance is a contract between you and your insurance company. You will be held responsible for any charges not covered by your insurance plan. It is your responsibility to know which providers are in your network, to understand your benefits, and to follow your specific plan’s requirements.
We will be happy to submit a claim to your primary insurance company. If you provide us with your secondary insurance information, we will file a claim to them as well, after the primary insurer has made its determination.
When surgery is necessary, we will confirm approval with your insurance company. We will discuss with you the terms of your benefits regarding the surgical procedure and your post-operative care which may, more often than not, include physical therapy.
Medicare
If you are a Medicare beneficiary, please bring your Medicare card with you, along with any supplemental insurance cards you have. Inform our registration staff if any other insurance coverage is primary and Medicare is secondary. Our services will be submitted to Medicare Part B, so if you only have Part A coverage, these services will not be paid by Medicare.
We are not currently enrolled in any of the Medicare HMO plans that are available, but we can accept any of the Medicare Private Fee-For-Service plans. Please inform our registration staff if you have one of these plans that takes the place of “regular” Medicare.
Worker's Compensation
When your employer is responsible for your account, please supply
us with the date of the injury, employer name and phone number
and
the name of the person who should be contacted regarding your injury.
We will verify the information and advise you if your injury cannot
be filed as a Worker's Comp claim.
Personal
Injury/ Auto Accident
If your injury is the result of an accident covered by auto or personal
injury insurance, each patient will be evaluated on a consultation
basis. This office will not accept assignment of fees based on case
settlement. Fees for personal, liability or auto injuries are due
at the time service is rendered.
You must supply us with all pertinent information.
